Becoming a patient is easy and straightforward. Just follow the instructions below. If you have any problems, please call our friendly staff and someone will help you.

To become a patient:
• Please call us at (206) 784-9111 to schedule your first appointment at a convenient time and location. We will answer any questions you have and help direct you to one of our exceptional naturopathic physicians.

• Once you have scheduled your appointment, please complete the following forms and fax them to us at (206) 784-7444. You may also mail them to:
