Becoming a Patient

Becoming a patient at Northwest Natural Health is easy.

Patient Navigator
To find out if Northwest Natural Health Clinic is a good fit for you, call (206) 784-9111 and ask to speak with our patient navigator, she will answer your questions about what we treat and what you can expect.  She can also schedule your initial appointment.  If your insurance provider requires it, you may need to ask your primary care provider (PCP) for a referral.  Regardless of whether or not you are referred to our clinic, we will coordinate your care with any and all health care providers you are currently seeing.

Convenient Locations
For your convenience, we have appointment times available in multiple locations around the Puget Sound.

Downloadable Forms
You can download our patient registration forms, or call us and we will mail copies to you. If possible, please fax or mail the completed forms to our office prior to your appointment. This allows the doctor to become familiar with your situation prior to your first visit. If faxed, please bring the originals to your appointment.
